Leave of Absence & Withdrawal & Change of Address

Please follow the instructions below if there is a change in the school register transfer and changes to the matters in a notification.
However, please inquire about details and an uncertain point concerning the notification at the student support group of your faculty (graduate school).  



 
Leave of Absence & Re-enrollment
Please submit the "Leave of Absence Application" form when you are unable to attend classes for more than 3 consecutive months due to illness or other compelling reasons.
Students may re-enroll by submitting the "Application of Returning to School" form when the reason for absence ceases to exist (even if the permit is still valid).

 Transfer Faculties
A student intending to transfer to another faculty should submit an "Application of Transferring to Another Faculty".

Please submit "Application of changing family name" in case your family name changed
Please submit "Application of using previous name" in case you wish to use your former (maiden) name.
