In order to prevent the spread of COVID-19, HU has kept a log of individuals entering and leaving the welfare facilities etc. This record will only be used if an infected person is identified and will not be used for any other purpose. As it will lead to saving lives, please be sure to record your entry/exit. We would like to ask for everyone's cooperation.

【Recording entry and exit via QR code】

①Please use your smartphone to scan the QR code posted on the entrance and exit of the facility when entering or exiting.

②Sit on a seat or move to a place with few people (when leaving the facility, leave directly).
* To avoid congestion at the entrance/exit, after scanning the QR code, please move to a space with few people, and then complete the procedure on your smartphone.

③The Microsoft Forms "admission record" will be displayed on your smartphone. Select the facility that you will use or that you have used, and then press the submit button.

* If the sign-in screen is displayed when accessing the answer screen, please enter the following information:
In the yellow section, enter your student ID number (letters must be lowercase).

If you cannot/could not use the system due to the reasons such as you do not have a smartphone, had trouble scanning the QR code, forgot to scan the card reader or could not submit your information due to trying to avoid a big crowd, please click on the entry/exit record shown in the table below. You can answer even after returning home, so please make a note of the facility you used and the time of use.
