How to apply for a Student ID-Card Reissue
In case of loss, theft or damage, please apply for the reissue of a new Student ID-Card according to the following procedure:
1. Contact the office of your faculty (or graduate school) as soon as possible. In principle, students applying for card reissue have to pay reissue fees by themselves. The way of payment will be indicated by the office of your faculty (or graduate school).

* Forms are available at the office of your faculty (or graduate school).
* First year students of the Faculty of Medicine, Dentistry or Pharmaceutical Sciences can also apply at the office of the Faculty of Integrated Arts and Sciences or Higashi-Senda Area Support Office.
◎Changing the identification photo
Follow one of the procedures described below and submit the photo to the office of your faculty (or graduate school).
① Pasting a photo on the document:
Print out the photo, affix it into the "Picture Box" of the "Student ID-Card Reissue Application" and submit the document.
② Submitting digital image:
Save the photo data on a USB memory device etc. and bring it to the office of your faculty (or graduate school).
*Assign the name of image file as "your student number(in one-byte capital letters)".
*The file extension must be designated in jpg.
* Before submitting the photo, read "Important Reminders for Submitting Photos" stated in the Student ID-Card Reissue Application.doc carefully.
2.To avoid abuse of electronic money or of the Hiroshima University CO-OP members card (if you are a member), report card loss as soon as possible to Hiroshima University CO-OP.
