Class Objectives
/Class Outline
To understand the integrated care system from health promotion to nursing care for those with health problems in the community, and to learn the functions and roles of the nursing profession. In particular, the students will learn how to provide continuous nursing support to those in need of health, medical, and welfare assistance, how to form multidisciplinary team care and networks with related professions and collaborating organizations, and how to form collaborative relationships in the community.

Please watch videos and website information listed during the lecture, and please read the assigned text books 
Requirements  
Grading Method Will distribute materials that describe the conditions for obtaining S, A, B, or C. Acquire the evaluation by properly performing and submitting the assignment for the evaluation you want to obtain (100%). 
Practical Experience Experienced  
Summary of Practical Experience and Class Contents based on it The lecturer worked as a nursing specialist at the Ministry of Health, Labour and Welfare, worked as a team member of establishment of the long-term care insurance system, and worked on improving the system development of home-visiting nursing system. The lecturers invited to each session are experts in developing integrated community care in the field. 
Message Includes fieldwork to actually explore and assess the area. Since the 3rd frame is secured, the fieldwork work may be extended to the 3rd frame.
