<Course Description> This is placed as an advanced clinical training of CNS course after finishing the first practicum. Apply all theories and concept studied at classes to clinical practicum. This practice is focused on community, in-home care at chronic, recovery, and end-of-life stages. Thus students are placed at a home-visiting nurse station and learn in-home care, discharge planning from hospital to home, care continuity, collaboration with other health professional, and the integrated community care system. Through this course, students will look out over the healthcare system in Japan. <Objectives> During this clinical practicum, the students will be able to; 1. Acquire knowledge and skills of a comprehensive health assessment for patients taken care of at home and the families. This includes assessment of environment, service-needs assessment, social resource assessment |
